Stitches and staples

Definition

Stitches and staples are two methods by which a wound may be closed. Stitches use specialized needles and thread to "sew" a wound closed. Staples are thin pieces of metal that are placed with a stapling device through the edges of a wound to hold it closed.

Description

Wounds to the skin, fat, muscle, blood vessels, and other structures in the body may occur accidentally (as in a cut) or purposefully (as in a surgical incision). A number of different methods exist to close a wound; the method selected depends on the type of injury, the type of tissue injured, the location and depth of the injury, and the patient's health. Stitches and staples are two commonly used wound closure methods.

Stitches

Sutures, as stitches are often called, are the way that most wounds are closed. Suture materials have various characteristics that determine their use. The two main components of suture materials are the needle and thread.

MATERIALS. Suture thread is often characterized by how long it retains its strength in tissue. Absorbable stitches lose their strength in a matter of days or weeks and are eventually absorbed by the tissue. This characteristic is useful for the suturing of subcutaneous tissues. Nonabsorbable stitches retain their strength for months to years and may never be absorbed by the tissue. They are generally used for skin and removed once the wound has sufficiently healed. Suture thread is made of various natural or synthetic components and comes in different diameters for use in different types of tissues. Very fine suture threads are used to close cuts on the face, while threads with a larger diameter are required for subcutaneous tissues.

Suture needles may resemble a conventional sewing needle with an eye through which suture material is threaded, or they come with suture thread attached at one end; this connection is called a swage. Needles may be straight or curved; the most commonly used shape is the semicircle, which permits easier manipulation through tissues by the clinician. Needles vary in length from less than 0.1 in (2 mm) to 2.4 in (60 mm). The point of a needle may be cutting (for such tougher tissues as the skin), rounded (for such easily penetrable tissues as the subcutaneous layers), or blunt (for such easily damaged tissues as the liver).

TECHNIQUE. While various stitching techniques may be used depending on the location of the wound and type of tissue to be sutured, basic suturing technique remains the same. Several instruments are necessary for proper wound closure, including dissecting scissors (for cleaning the wound); suture scissors (for cutting suture thread); a needle holder (for manipulating the needle); and forceps (for manipulating tissue). Wounds resulting from an injury must be cleaned before closure; dead tissue and foreign bodies are removed and the area is cleansed with an antiseptic. Sutures may be interrupted (each stitch is separately placed, tied, and cut) or continuous (one continuous piece of thread composes all the stitches); they may be placed at different angles and depths.

Nonabsorbable stitches should be removed several days to weeks after their placement, depending on their location. For instance, sutures on the face should be removed in approximately 5 days; sutures on the legs and abdomen, in 7 to 10 days; and sutures on the back, in 10 to 14 days. Strips of adhesive tape may be placed over the wound to help support the tissue while it is healing.

Staples

A distinct advantage that staples have over sutures is their quick placement—stapling is approximately three to four times faster than suturing. Staples are also associated with a lower risk of infection and tissue reaction than sutures. It is, however, more difficult to correctly align the edges of a wound for stapling, and staples generally cost more than sutures. Common locations of wounds that may be stapled are the arms, legs, abdomen, back, or scalp; wounds on the hands, feet, neck, or face should not be stapled. Additionally, staples may be used to connect cut ends of larger blood vessels or segments of the bowel.

MATERIALS. Individual staples are composed of stainless steel and have a crossbar that lies parallel to the skin, two legs that enter each edge of the wound, and tips that hold the staple in place. Staples are placed with the aid of a stapling device that generally holds between 5 and 25 staples. Forceps are also necessary to help align the edges of the wound together and hold them in place until staples can be placed.

TECHNIQUE. The wound is first cleaned of dead tissue and foreign bodies and washed with an antiseptic. The edges of the wound are aligned and held together with forceps or the clinician's fingers. The stapling device is held against the wound at the point at which the staple is to be placed. By squeezing the trigger on the stapling device, the staple is automatically placed into the skin; the depth of placement is controlled by how hard the stapling device is held against the skin. The staples should be removed in approximately the same time as sutures; this is done with a specialized staple remover.

No. It does not hurt when the staples are removed. It is a simple thing and not painful at all. It is very quick also. Nothing to worry about

My wife had a surgery one month ago(arthroscopy)in the right shoulder. The hospital charged us US$1,200 only for the suture, which is described in the account as "Suture Grasper 60º Ref 251723 - Depuy" .
These are golden stitches?
WhaT TYPE OF THREAD AND NEEDLE WORKS BEST FOR SUTURING LIKE A HAND OR LEG WOUND?

My mom just had staples done on her laparotomy wound. I hope they wont be painful while removing.
What kind of tool do they use to remove staples and is it a painful process? Is any kind of sedation required for this procedure?

I think staples are being used more and more because surgeons are getting lazy! Look at a wound after stiches, and look at a wound after stapled. The stapled wound is ugly and the stiched would looks, well, not 100% but MUCH better than the stapled wound. It takes skill to suture a wound! I have never had surgery, but my wife has. I will NEVER sign a consent form allowing staples if I ever get surgery. We done fine for decades with regular stiches! My body will not be scarred because a lazy surgeon wants to hurry back to his extravagant lifestyle!

Hi, I'm 20 years old and I just had a brain tumour removed, about 12 days ago. I had 40 staples in the back of my head. They took half of them out yesterday, and the rest come out tomorrow. They don't hurt, except the nurse pinched my skin and it bled a little but it's fine :) It took some strategic shampooing with the help of my mother (to avoid the staples getting wet.) and you just have to avoid getting the staples wet. Basically, my head was over the sink and my mother was shampooing my hair for me ahaha. Just be patient, there isn't any nasty scaring with the staples.

So Are You Not To Wash Your Hair When You Have Staples In It, Because One Of The Doctors At Baptist Hospital In Columbia SC, Told Me I Can Wash My Hair After 24Hours, Because The Wound Would Be Closed Then?

Staples aren't necessarily used because of laziness Jeff and I don't believe there's a section on consent forms where you can request the type of wound closure you want, so that is just ridiculous to me. I've personally had both staples and stitches and the same operation done twice, the first time with stitches, second with staples. Staples have a much stronger hold and are used in particular for long wounds and wounds in an area of high movement, they offer an equal pressure and can be easily spaced, they don't however necessarily cause a worse scar, so many other factors are involved in making what a scar looks like and more often it's the stitches being stitched too tight or too loose, not evenly spaced, busted open because of the area of the wound being moved. I've seen terrible scars much worse caused by stitches than staples being a nurse, staples also are usually easier to remove and don't cause the patient too much discomfort as stitches can, staples are also easier to been seen and don't break therefore making the whole foreign body easily removed where as stitches can break and can be mistakenly left behind causing more infection potentially. Also most surgeons don't close wounds anymore and haven't done for decades so it's not laziness attributed by them, they have registrars, assistants and closing nurses to finish the job, so really each wound is closed with the most appropriate method for the situation! You should really see what a scar from a wound needing over 100 stitches looks like, it's far less prettier than what staples do to it!

I had nearly 30 staples after leg surgery and when I went to the surgeon's office to have them removed he deferred to the MEDICAL ASSISTANT, who removed them without him being present. I later found out the limits of the duties of the MA do not include this level of care in their job description. Fortunately, I have not yet identified anything of lasting damage with regard to this misappropriation of personnel. Nonetheless, is it ethical?

I had two tumors removed, I have a total of 96 staples, 28 in my back and 68 in my leg. They are supposed to come out in three days after being in for just short of a month. The visiting nurse suggested that I take pain killers before the staple removal. Is this really needed? Is it painful enough to need that precaution? The incisions look good, no trace of infection.
I got ten staples in my head, because I got hit with a brick. I had to wait for only a week to get removed by a doctor, but because it was bothering me. I only removed 3 staples. Can my head get an infection if I keep removing it. Would this raise a cost of a medical bill?

I had a hernia surgery in July 2015 and had 42 staples. The doc said he had to staple because my situation was grave and he didnt have much time...staples take less time. They were removed after a week. There wasnt much pain, except for one or two that were bent inside. Problem is, the end of the wound was not quite closed and started weeping. And the scar is not very pretty...small price to pay for having my life saved..my hernia had burst they had to cut 30cm ofmy intestines.

My mom had oral surgery for removal of cancerous tumor in buccal cavity and the doctors had used a patch to cover up the wound. Now after 6 months of surgery she still experiences severe pain and an OPG shows that she had over 25 stapler pins internally where the surgery was performed. Is it likely that these staple pins are causing the pain, and since many of them are in deep tissue, what procedures would doctor perform for their removal. Kindly advise.

I had hip replacement in Cartagena Spain a week ago. My insurance is arranging to fly me back to Arizona in a few days. I am 69, with no cardiac, vascular, or respitory issues. There have been no complications and I will be accompanied by a traveling nurse for entire lengthy trip. My question: I have 25 staples in left hip area. Is it safe to fly with these staples still in, or should they be removed? The MD has certified me "fit to fly" and I do not wish to delay going home any longer?
Thank you.

Hi, I've had an ACDF. ive just discovered I have 14 staples in my neck. The nurse told me upon discharge that I had one big suture, and all my discharge paperwork states sutures. Ive read that staples shouldnt really be used in the neck ..I don't see my consultant now for another 6 weeks..
Will my scar be worse as I have staples, is it right that they shouldn't be used??

Went to the ER by ambulance with a "hurt hip". Found out it was broke and was immediately took to surgery. Now that it's time to have staples removed the Dr.'s office wants to charge me $300 to have my staples removed. I told the ER that I had no insurance when I came in and they got me on a special program that the hospital offered for people like myself. Needing my staples removed and called the Dr. Like instructed to to make a appointment to have staples removed and got turned down because I don't have the money to pay the office visit. Now what do i do??? No money no service...well the doctor knew when he operated on me so what's different now? Should I just leave them in so I can get an infection and then sue his ass?? Where do I go now to have someone complete his work? Is he liabable to remove the staples even though I have no money, so im stuck just like my staples are. Please help!!

I recently was injured while chopping out my bike. A cut off wheel had come apart while I was using it on the grinder. I received eleven staples in my leg and another ten in my arm. The wound on my leg was deeper into my flesh and muscle. I have had the staples removed and now it's coming open and the Dr. refused to do anything about it. The staples where done in a very unprofessional manor to the point where the staple would just be tacked into one side of the wound. Now I have to deal with it being infected open and painful. What can I do to quicken the healing process
Boyfriend was in car accident. Had to have staples in and around upper part of his head. Hospital said 9 to 10 days for removal of staples. It will be 16 days before the staples will be removed. Will this be ok? Also, he has stitches around side of his left eye. Will this be ok for same duration of time, 16 days vs. 9-10 days before removal? Thank you for your reply!
Happy Thanksgiving!

I am a fifty five year old male that suffers from folliculitis pillar cysts. The doctor performed a biopsy of the area, the stitch was non-dissovable around two inches long. It was done right above the ear. The day before it was to be removed it became loose, I called the doctor and told me all was good since it was closed and not bleeding. Problem is the next morning the stitch was gone completely, since it was for a cyst it was semi deep, the doctor said all was good no worries and didn't even attempt to see if still under skin. I now suffer from painful sores that appear to be filled with fluid. I pulled out a hair that was ingrown from the area that was like a needle stabbing me all day. When the hair came out it was covered with oily puss. There was so much fat on one end of the hair it left one mm hole that started pouring a dark maroon oily blood down my face into the sink, I closed drain to attempt to measure. It was about four to five table spoons. It was not like normal blood, you could actually pick it all up with a pair of tweezers. Doctors act like I am crazy. The headaches are horrible when the area swells. It has gotten to the point where I have to actually take a sterilized needle and release fluid from time to time with fluid amounts always about the same size and type, some are even yellowish. What could the long term effects be? I am worried this will one day cause a severe infection. Why do the doctors not care? What are my options? I keep hoping and praying that one day I could see the black stitch under the skin or better sticking out. They just want to give me antibiotics which I am not a fan of because the damage they do longterm to the immune system.

Well I ask the doctor questions before I got staples because I’ve never had staples in my head before now , I asked if they numbed it before he did it and he said well on the head not really then I supposedly cleaned it and the doctor realized that it was worse than he thought it was so he told me was going to numb it and put more staples it’s been eight days ,I wound up still only getting three staples and I’m not a doctor but I need at least five maybe six and I still had matted blood on the staple wound Not to mention they stapled my hair down with the first two Staples.I don’t think they’re healing e like they should enough anyways and last night for the first time in the first staple one side they had pus coming outI just want to know what is the loans to Staples can stay in there and is plus a bad song it was a much I would like to post a picture but I couldn’t find a way to post it because I took a picture immediately after they put three staples in and it look like I just got in an accident because that’s hell not clean that they did it I want to going to another hospital after and he said that I need to cover more staples but by that time it was too late

I am allergic to dissolve stitches, so my Surgeon opted for staples. How long is too long to keep them in? My surgery was on December 10 and surgeon is on vacation for the Holidays and wants to remove on the 4th of January. They are irritated. I worry this may be too long to wait. Online it says 14 days is usually the maximum. (umbilical hernia)

I had surgery almost 2 weeks ago. I had to have pins put in my knee. My surgeon used staples to close my knee up. I was wondering if staples hurt and also will I be able to bend and use my knee better afterwards?
Had total left knee replacement at the V.A. in 2018 and the doctor left 8 to 12 subcutaneous non dissolvable stitches in my knee. My knee is very sloppy there's movement of parts with some scary sounds and chronic pain. Have developed intense heel pain in my right heal.

Hi I was in SFGH and had a procedure done to close a large open wound from a wine bottle someone hit me and they gave me staples with a wall stapler and they left some of them in after I went to my appointment to have them removed and aproximately a year later I found my head was bleeding again in the shower and I felt there and these twisted metal staples were still in my head, I was shocked and they were all rusted and I had caught Hepatitus C from the procedure and luckily got cured of that from the Harvoni. Is this uncomman and I filed complaint and the hospital apologized after an investigation and they gave me a letter for the City Controllers Office Claims Division, I had to file a late claim because the 6 month had lapsed and well I hope I get compensated Because they incorrectly put a catheder in and have damaged my penal area I am dripping all day long and have hardly any feeling there, smelly problem and numb all the time. What I think is I am glad I survived however I am not the same and what I wonder is how do I feel knowing that they made an error with the regular staples and not some ferous metal that wouldnt rust. Thanks for the info

Our 6 yrs old son had a head injury last March 12, took him to a well known pediatric urgent care, the nurse asked me what procedure I prefer, suture or staples. Since this is not my line or skills, I asked her if it’s her child what procedure she’ll pick. She said staples, so we go ahead with it. ‘Twas traumatic for our son, screamed the whole time when staples is being put in place, but he was back to his normal perky self as soon as we left the hospital. I followed the instructions on how to care for his wound, no infection and it’s healing as we expected. After a week we came back for the staple removal. I did searched on how they remove the staple, from the video that I watched, it seemed so easy and not painful. But not for our son, somehow the nurse was having a hard time removing the first staple, at this point our son is already screaming and kicking since it’s been 20-30seconds that the nurse is pulling his head, I wondered why and what’s wrong, then she realized the tool/plier she’s using to remove the staple Is defective. So she went and get a new one, but our son is already freaking out at this point, he doesn’t want anybody touching his head again. A male nurse came to helped us hold him still, especially his head as we proceeds with the removal. Painful and traumatic experienced which I think it shouldn’t have, if they’ve prepare well before the procedure. I can see that the would is healing as it supposed to, which is good. We’re concerns about the scaring of how it will look like, but that really varies they said in each person, of course everyone is different, How you take care of it/genes/hormones etc. So they can’t really tell if it will be ugly or not. He feels itchy On the areas where the incite is , but it’s normal as it heals. But tonight my son complains suddenly of sharp pain in his head, 3 times it striked him as I’m reading to him during bed time. He was tapping his head and said its hurts so bad. Now I am concerned a bit. They rule out concussion since they’ didn’t see any signs or symptoms of it when he was brought the first time. I think I’ll call them to make sure this is nothing serious .

I just got staples on my head. Does anyone know if its okay to sleep with them? As in laying down on the wound. It hurts putting pressure on it and laying down to sleep has become a challenge. I'm afraid I will tear them off in the middle of the night
Is it normal for a tiny amount of blood and serum to come out of the holes when the suture thread is pulled out during removal of sutures? The wound is otherwise healed and closed well. Could there be a covert infection underneath?

I have a surgery done on 2012, to remove my ovaries. I remember after the removed the staples I was having lots of pain. I went back to the Dr. who did the surgery and told her I think a staple is still inside me. She don't believe me. She said can't be who removed all of them. Guess what happend to me as of today july19 2020,I still have the staple inside me. One day I went for a bone density test and the saw the staple. My question is can i go for a mri having that staple inside. The other question should the staple be removed. What can happend is not removed.
